The Problem Worth Solving
A Malaysian healthcare network was struggling with fragmented patient data across more than 200 physical locations. Each clinic ran isolated on-premise servers using three different legacy EMR systems. Cross-clinic patient history could not be accessed in real time, leading to repeated diagnostic tests, avoidable medication risk, and significant administrative overhead — a single record transfer could take days.
How We Thought About It
We began with a six-week data audit across twelve representative clinics to catalogue schema differences, data quality issues, and regulatory constraints under PDPA Malaysia. We designed a phased migration strategy — starting with a read-only unified view before enabling write-back — to eliminate downtime risk for live clinics. The cloud architecture was designed with a private VPC, end-to-end encryption, and audit logging to satisfy both PDPA and international standards.
What We Built
We built a custom middleware layer to safely extract and normalise legacy data from three disparate EMR systems into a centralised PostgreSQL cluster. A React-based portal gave clinicians instant read access to unified patient histories across all sites. Write-back capability was rolled out clinic-by-clinic over three months to minimise disruption. Role-based access controls ensured only authorised clinicians could view or edit records.
What Changed
Clinicians no longer wait on manual record requests, and the administrative work of chasing records between sites has largely disappeared. Decommissioning the decentralised on-premise servers removed a recurring infrastructure cost. The phased rollout completed without a single clinic losing a working day.
